Are my wishes clear?

People are often unwilling or unable to discuss the prospect of incapacitation or death, even with those who are closest to them. If a serious health issue presents itself, and a person can't speak for herself, there might not be anyone who knows her wishes.

I have photographed and filled in what I consider to be the relevant/important points to focus on from a DIY Enduring Power of Attorney and Advance Health Directive pack, with a few personal changes and additions. I have roughly pieced it all together. I might type up a more polished or cute copy later.

I would also like to state that if I am in the kind of condition that requires an advance health directive, I would not want anyone to visit me in the hospital, except if it was necessary to ensure that my wishes were respected.

While I'm here..

If I die, I would like to be cremated. I do not want to have a funeral. GK and I have already discussed the possibility that he would go on a scattering mission. My clothes can be donated. If anyone feels that there is anything unresolved between us, it is best to contact me before I die. I could die at any time. I don't have much, but there is a possibility I will spend it all in one last spree.
